Eligibility for the Medicare Part B program is offered when the patient qualifies with a diagnosis that
justifies medical necessity.

HOW TO ORDER MEDICAL SUPPLIES/DIABETIC SHOES & More:
1. Your Doctor will give you the Prescription, or can fax prescription order to us with following information: Primary diagnosis, Length of need of items (1-99 indefenite need), Quantity of items needed, Date of Face-to-Face examination and current progress notes/chart notes, PT reports, etc., along with with patient's face sheet name, address, D.O.B, phone number, SSN and Primary and secondary insurance information.

2. Once we receive the order we will verify eligibilty, if any co-pay will have to be payed at the time order is render.

3. We will call you to schedule delivery of supplies, set it up for you, and train you on how to use our products.

4. We will bill your primary insurance.
